We would argue that further studies with chronic administration of low doses of nicotine are required to clearly evaluate its impact on carcinogenicity. In this line, a study compared the acute impact of CS vs. e-cigarette vaping with equivalent nicotine content in healthy smokers and non-smokers. Both increased markers of oxidative stress and decreased NO bioavailability, flow-mediated dilation, and vitamin E levels showing no significant differences between tobacco and e-cigarette exposure (reviewed in [20]). Inasmuch, short-term e-cigarette use in healthy smokers resulted in marked impairment of endothelial function and an increase in arterial stiffness (reviewed in [20]). Similar effects on endothelial dysfunction and arterial stiffness were found in animals when they were exposed to e-cigarette vapor either for several days or chronically (reviewed in [20]).

This health advisory is being issued by the State Health Officer to inform the public about the alarming statistics on current e-cigarette use among youth in Wisconsin. In Wisconsin, current e-cigarette use among Wisconsin high school students increased 154% between 2014 and 2018.

More people began vaping in 2023 than initiated alcohol or illicit substances. A 2015 study suggests that people who vape nicotine are more likely to become dependent on nicotine than people who vape nicotine-free fluids. When studying tobacco cigarettes, researchers rely on smoking machines that simulate how frequently a typical smoker takes a puff and how much smoke is inhaled with each breath. No one has yet determined how much e-cig vapor the typical user breathes in, so different studies assume different amounts of vapor as their standard, making it difficult to compare their results. Tracing what happens to that vapor once it is inhaled is equally problematic.
Replacing your cigarette with a cigar, pipe, e-cigarette, or hookah won’t help you avoid the health risks. The U.S. government has taken steps to restrict most flavored vaping products, effective in 2020. While this should reduce the number of flavored cartridges available in the United States, the guidelines are limited to products submitted for market authorization and don’t include those made at home or purchased online from other countries. Other studies suggest that while using e-cigarettes sends pollutants into the air, the concentration of these toxins drops quickly after someone stops vaping (much faster than cigarette smoke), but that doesn’t mean that the risk has gone away. Some, for example, will settle on surfaces, such as carpets or furniture, where small children might touch them or inadvertently ingest them when placing contaminated objects in their mouths.
Research on health effects of secondhand exposure to e-cigarettes is still emerging. The quantity of these carcinogens is less that what is found in conventional cigarettes. E-cigarettes are battery-operated devices used for a type of smoking called vaping.
This ban excludes disposable e-cigarette devices, which are sold in many appealing flavors and are relatively inexpensive. And are now the most popular type of e-cigarette used by adolescents. Nicotine is highly addictive and may lead to nicotine cravings and development of tolerance (the need to use more often in order to have the same effect). Most have a battery, heating element, and place to hold a liquid, usually containing highly addictive nicotine, that is added or included in the device. The heating element aerosolizes the liquid for the inhalation of the liquid nicotine or other substance. E-cigarettes are called many different names, including "vapes," "e-cigs," "vapor products," and "electronic nicotine delivery systems (ENDS)." The chemicals in electronic cigarettes, commonly called vapes, can severely damage the lungs, according to studies reviewed by the American Lung Association. The nicotine in them, which can be in higher quantities than traditional cigarettes, can also impact a child’s brain development. Several peer-reviewed studies found that nicotine negatively affects teens’ memory and attention and can increase their risk of developing other mental and behavioral problems later in life.
In contrast, other studies found acute microvascular endothelial dysfunction, increased oxidative stress and arterial stiffness in smokers after exposure to e-cigarettes with nicotine, but not after e-cigarettes without nicotine (reviewed in [20]). In women smokers, a study found a significant difference in stiffness after smoking just one tobacco cigarette, but not after use of e-cigarettes (reviewed in [20]). However, research updates on, and analysis of, the potential health benefits/risks of e-cigarettes are outside the scope of TobaccoTactics. These devices come in a variety of shapes and sizes, as pictured below, with several different names. Some e-cigarettes look like regular cigarettes, but many resemble pens or USB flash drives, and are not recognized as tobacco products. Larger e-cigarettes such as tank systems, or "mods," do not resemble other tobacco products.
Dual use, which is common, is at least as dangerous and likely more dangerous than smoking conventional cigarettes or using e-cigarettes alone. Further, not all ENDS are the same and the risks to health may differ from one product to another, and from user to user. E-cigarettes are still fairly new, and more research is needed over a longer period of time to know what the long-term effects may be.

However, no rigorous scientific studies have shown that they are safe for use. Some people who smoke choose to try e-cigarettes to help them stop smoking. But switching to e-cigarettes still exposes users to potentially serious ongoing health risks. It’s important to stop using all tobacco products, including e-cigarettes, as soon as possible both to reduce health risks and to avoid staying addicted to nicotine.
However, the long-term effects of all flavour chemicals used by this industry (which are more than 15,000) remain unknown and they are not usually included in the product label [78]. Furthermore, there is no safety guarantee since they may harbour potential toxic or irritating properties [5]. In addition to its toxicological effects on foetus development, nicotine can disrupt brain development in adolescents and young adults [44,45,46]. Several studies have also suggested that nicotine is potentially carcinogenic (reviewed in [41]), but more work is needed to prove its carcinogenicity independently of the combustion products of tobacco [47].

It would seem that future studies should analyse the possible toxic effects of humectants and related products at concentrations similar to those that e-cigarette vapers are exposed to reach conclusive results. Vape aerosols are not just harmless water vapors that are inhaled and exhaled (as marketed). Vape aerosol exposure is unsafe and contains chemicals, metals (i.e., lead, nickel), and other particles that can interfere with lung development and health.
One chemical, known as 6-methyl nicotine, has been shown in rodent experiments to be far more potent than nicotine in targeting the brain’s nicotine receptors and more toxic than nicotine. Smoking harms nearly every organ in the body ― heart and blood vessel disease; lung disease; cancer (almost anywhere in your body); and impacts to your bones, eyes, teeth, gums, fertility and pregnancy. There are different types of e-cigarette, including vape pens, vape bars, pod devices, mods, and cigalikes. Its application has been reported to improve heating efficiency and reduce charring14,16,18,19,20. Additional research is ongoing to clarify the dangers from vaping, and how vaping damages the lungs and other organs. Long-term human studies, in particular, could bring the risks from e-cigarettes into better focus, Crotty Alexander says.
Australia, New Zealand, Poland, and Pakistan have a nationwide ban on the selling of all tobacco products to people under the age of 18. "Since most of the health concerns about e-cigarettes have focused on nicotine, there is still much we do not know about e-cigarettes. The present study utilized a nationwide school survey which covers the majority of the respective Finnish student population and a validated measure for susceptibility, adapted to cover also e-cigarettes and snus. The limitations include self-reporting, lack of class-level data and a measure on school performance, and potential bias in parental education and smoking when reported by students. Further, no causal conclusions can be made from this cross-sectional data. Some adolescents experience first dependence symptoms early [2], which makes prevention of nicotine use essential.
Probably the worst thing a parent could do for their child would be to buy an e-cigarette under the misconception that this might prevent them from smoking regular cigarettes, Krishnan-Sarin says.
